White Highlights vs. The Skin

I have been working with a lot of black and grey recently and was just wondering what some other artsists' take was on the usage of white highlights as aposed to using the skin itself as the highlights like using the white of the paper on a drawing.. for example in fabric, hair ect. How do you use it, or do you not use it at all? Thanks a lot, Cutter
